Accident Report Guide: How and Where to File
Filing an Online Police Report
If you answered "YES" to all the following statements, you may file a police report online:
- The accident occurred in New Jersey.
- The accident involved two vehicles.
- No one was injured in the accident.
- There was no significant property damage.
To file an online police report, follow these instructions:
- Go to the New Jersey State Police website: https://www.njsp.org.
- Click on the "Online Services" tab.
- Select "File a Police Report."
- Enter the required information and submit the report.
Requesting a Copy of Your Accident Report
There are three ways to request a copy of your accident report:
Online
You can request a copy of your accident report online through the New Jersey Motor Vehicle Commission (MVC) website: https://www.state.nj.us/mvc.
By Mail
You can request a copy of your accident report by mail by sending a written request to the following address:

West Trenton, NJ 08628
In Person
You can request a copy of your accident report in person at any New Jersey State Police barracks.
New Jersey Self-Reporting Crash Form SR-1
The New Jersey Self-Reporting Crash form SR-1 is used to report a crash that was not investigated by the police.
To obtain a copy of the SR-1 form, you can:
- Download the form from the MVC website: https://www.state.nj.us/mvc.
- Request a copy of the form by mail by sending a written request to the following address:

You must complete and submit the SR-1 form to the MVC within 10 days of the accident.
